Program Overview

The Transition Out Of Poverty program (known as T.O.P. or Case Management) is funded by Community Services Block Grant (CSBG).  This program assists low-income individuals and households transition out of poverty by working to strengthen areas of our clients’ lives by providing information, resources and referrals in their pursuit of short- and long-term personal, professional, and financial goals.

Income Eligibility

The client’s income should be no greater than 125% of the federal poverty level.  To determine income eligibility, all income sources from the individual or household are collected for a period thirty (30) days prior to the date of application, then annualized.  Income documentation is collected from all income sources for all household members eighteen (18) years and older for the entire thirty (30) day period.

Required Documentation

The documentation requested to capture household demographics include photo identification, Social Security cards or numbers for each family member, and proof of income 30 days prior to appointment date (for each household member 18 years or older).

Counties Coverage Map

This program is aimed at low-income residents in Bailey, Cochran, Crosby, Dickens, Floyd, Garza, Hale, Hockley, King, Lamb, Lynn, Motley, Terry and Yoakum Counties.  Interested clients should contact the neighborhood service center located in their county and schedule an appointment.
