Community Based Alternatives
This program privides home- and community-based services to people who are ederly and to adults with disabilities as a cost-effective alternative to living in a nursing home. DHS employees provide case management services.
Services are available across the state; however, the average length of time on the interest list varies by where you live.
Services may include:
- Adaptive aids and medical supplies
- Minor home modifications
- Occupational therapy
- Personal assistance services
- Physical therapy
- Respite care
- Speech pathology services
- Nursing services
Minimum program requirements
To be eligible for this program, you must meet the following criteria:
- Be age 21 or older.
- Be a Medicaid recipient or be otherwise financially eligible. Eligibiltiy is based on the income and resource requirements for Medicaid nursing facility care.
- Countable income must not exceed:
$1,692 per month for an individual;
$3,384 per month for a couple (both spouses applying).
- Countable resources must not exceed;
$2,000 for an individual;
$3,000 for a couple (both spouses applying). Spousal impoverishment provisions apply.
- Have a medical necessity determination.

Clients must first call your local DHS office for screening. DHS will contact the agency of your choice following eligibility determination. Services will be initiated following DHS referal, assessment by a nurse and final medical necessity approval.
